Locate file to upload:

Document size limit is 4 megabytes.

File names cannot contain any special characters (for example: / , - _ + & ! ' *). Use a simple file name like: JDoeGradApp.doc. An error message will appear if your file name is incompatible. Remove passwords and encryption from protected documents prior to uploading.

Typed and digital signatures are not accepted. You must download and sign the form. View locations of available scanners on campus.

Form 1040, 1040A, and 1040EZ must be signed by at least one parent. If a third party prepared your tax return, the name of the preparer must be included on the form along with the Preparer Tax Identification Number (PTIN) or Employer Identification Number (EIN).

You can confirm that we have received your upload in MyCUInfo 72 hours after submission by checking the “status” of the to-do item. Statuses are available under each to-do item under Holds/To-Do Items.
